Understanding Your Breast Augmentation Options Incision options can be discussed with patients in a breast augmentation consultation, but they will be made in inconspicuous areas to hide scarring. You have many options when it comes to breast implants and this is a decision we will make with you. Your breast implant size and type depend on multiple factors, such as your desired increase in size, but also on your breast anatomy, skin elasticity, and body type. There are many resources to help you evaluate and understand your options with breast augmentation, but sometimes it’s hard to know which source to trust. We can help answer any questions or address any concerns you may have. What Is The Procedure For Breast Augmentation? Breast augmentation with Salt Lake plastic surgeon Dr. Paul Watterson involves many decisions prior to your actual surgery. You’ll decide the type of implants, the size and projection of those implants, the incision location, and if you want the implants placed above or below the pectoral muscle. For the actual breast enhancement surgery, you’ll be placed under general anesthesia. After the incision is made, the next step is to create a pocket for the implant. If you have opted for saline implants, the near empty shell is placed in the breast pocket and is then filled. Silicone implants are placed into the pocket fully filled. The size of your silicone implants can limit your incision options. Once both implants are placed, your surgeon will check for symmetry and balance. Then the incisions are closed. The procedure is not overly difficult and your recovery is easier than you would assume. Breast Augmentation FAQs Do I Need To Be Done Breastfeeding? Yes! We want to have your breast size at normal for you so we can size you appropriately at surgery. A good suggestion and guide is to wait 3-6 months after breast feeding. What Type Of Results Can I Expect After My Surgery? Fantastic results of course! When we carefully assess and set expectations accordingly, you can and will have a great outcome. Initially, you will be swollen from surgery and the implants can “ride” a little high at first, but usually settles down after six weeks. Your pain won’t be horrible either, because we will place a long acting anesthetic in the pocket to help. After a day or so we will place you on ibuprofen and that works amazingly well for pain control. Can Breast Augmentation Be Combined With A Breast Lift? If you have a good deal of sagging skin on your breasts, either from pregnancy, childbirth, and breastfeeding, or from the gain and loss of a significant amount of weight, you’ll probably want us to perform a breast lift with augmentation. This removes the excess sagging skin prior to placing the implants. Adding implants alone to sagging breasts is not recommended. When Can I Resume Normal Activities After Breast Augmentation? Recovery from augmentation surgery is not difficult. For the first few days you will have some discomfort, but most patients don’t require heavy-duty pain medication. At first, your chest will feel tight, but this is perfectly normal as the tissue needs to stretch and the implants need to settle into place. When you first look at your new breasts, they may appear too high on the chest. But this is also normal, as settling will bring the implants down somewhat. Most patients can return to work in just four or five days. You can begin non-impact activity in a week or so, but impact exercise needs to wait for at least one month. Specific upper body exercises can resume in six weeks.

Women come to Watterson Plastic Surgery with different breast needs and different expectations—and one of the first steps we take is to sit down with each patient to ensure they’re pursuing the correct treatment. For example, for women who want additional breast volume, the right procedure is breast augmentation. A breast lift doesn’t add volume; its purpose is to change the shape of the breast. Of course, many women pursue breast augmentation and a breast lift at the same time. For women who pursue a breast lift, or mastopexy, there are different types to consider. Some women only require a slight lift with a minor incision; for other women, a more involved lift with a significant incision is required. Your plastic surgeon will consult with you on the type of breast lift that’s right for your needs. Breast Lift Vs. Breast Augmentation Breast lift or mastopexy is very different from simple augmentation. Augmentation does not really lift the breast. That is a bit of a myth. Sure, the breast skin will be tighter with augmentation, but that can relax over a very short time. In fact, when an augmentation is done to lift the breast, you can usually count on a less than desirable outcome. It can lead to a very disappointing outcome. There are a number of factors that go into a decision to lift a breast and even more factors that go into deciding whether to lift and augment at the same time. Stretch marks or striae, postpartum involution and decrease in breast size, and nipple position in relation to the breast are just a few. Most women understand the word “sagging” as it relates to their breasts and when I hear that in a consult before examining the patient, I can usually predict the patient will need a lift. If a patient’s breast can hold a pencil in the inframammary fold that is another simple do at home test that can predict the need for a lift. Usually, this is a very simple and easy decision, but occasionally there are borderline cases, and this again requires a consultation and discussion about expectations and outcomes so the appropriate decision can be made. There are three basic types of breast lifts, the circumareolar (around the nipple areola), the lollipop or circumvertical, and lastly the anchor or (wise pattern) type. We can help you decide what is best for you. It is very important to decide whether an implant should be used with a lift. A small or even a large implant will give “upper pole” fullness that is almost impossible to get any other way. Many women crave that upper pole fullness they used to have while breastfeeding or before children. This is why we often combine augmentation with a lift to achieve that look. Again, it is so important to team up with a skilled and experienced surgeon who can guide you in these decisions. What To Expect From Breast Lift Surgery A breast lift is intended to accomplish three distinct goals: First, the breast itself is uplifted, reducing the overhang of the breast—allowing for the underside of the breast to be more visible. The nipple is elevated to a more central location on the breast mound, which visually enhances the shape of the breast. Finally, a breast lift restores upper fullness to the breast by moving the tissue upward. These are all positive effects that can help women achieve the desired outcomes. With that said, many women also have concerns about breast surgery of any kind. One of the chief concerns is visible scarring. This is a natural anxiety for any woman to have as she considers a breast lift, and Dr. Watterson can best relieve that anxiety by showing real patient breast lift before and after photos—demonstrating how minimal scarring tends to be, and what it ultimately looks like over time. Another common concern is recovery time. How long will it take the average patient to recover fully from a breast lift procedure? The procedure itself tends to take about two hours, and after that time the patient will spend about one hour in the recovery room. Following that, the patient returns home. There is minimal discomfort associated with a breast lift and it’s typically resolved in just a few days. From there, recovery is a simple matter of letting the body heal.

  Why Get Breast Reduction Surgery? Oversized breasts can cause a host of problems, including neck, back and shoulder pain, skin irritation, and emotional distress. These are problems that not only affect how you look but also how you live your day-to-day life. If you have macromastia (oversized breasts), you can be limited in not only your choice of clothing but also the activities in which you can participate. Some patients can also develop yeast infections and other skin irritation problems underneath the breast (within the skin fold) that may require continuing medical treatment. What Concerns Can Breast Reduction Surgery Help Treat? Some common issues breast reduction patients face are: Back, neck and shoulder pain that persists even after medical treatments such as medications, massage, and chiropractic care Indentations or deep grooves in the shoulders from bra straps Restricted physical activity, inability to run, or general discomfort being active Skin irritation including but not limited to yeast or fungal infections in the breast crease that may require topical creams or ointments Sagging, stretched breast skin A low breast profile where the nipple-areolar complex rests below the breast crease Enlarged areola caused by stretched skin Although breast reduction surgery is frequently performed to address medical issues, it may also be done electively for patients who are simply unhappy with the size of their breasts and desire to have smaller breasts. What Is Breast Reduction Surgery? Breast reduction (or “reduction mammoplasty”) involves the removal of excess breast tissue and breast fat to reshape, resize, and lift the breasts. This procedure helps the patient to achieve a breast size that is more balanced and proportional to the rest of her body. It also relieves the mental distress, physical restrictions, and painful strain on other parts of the body caused by excessively heavy breasts. The Breast Reduction Procedure The incisions used by Dr. Watterson will depend on your unique condition. Usually one of two incision methods is used, dictated by the amount of sagging you’re experiencing. These procedures are performed under general anesthesia. Here are the two incision options: Traditional Incision Method — If you have extremely large breasts, we will likely use the traditional incision method. This allows the most access to remove sagging skin and tissue. The incision, also called the “anchor incision,” circles the areola, runs down to the breast crease, and then follows the breast crease a few inches in each direction. This is the most common procedure. Vertical Incision Method — This incision follows the same path as the anchor incision, without extending along the breast crease. The shape of this incision is more like a lollipop. It can work for women with moderately large breasts. This technique is used more in lifts with implants. In both methods, excess glandular tissue, fat, and skin are removed. The nipples and areolae are usually moved to a higher position on the chest. The areolae can also be reduced in size. Breast Reduction Recovery This procedure involves significant tissue reduction and repositioning. This creates tissue trauma, which results in swelling, bruising, and soreness. After your surgery your breasts will be fully bandaged and you’ll be wearing a surgical bra. You’ll have to take it easy for the first one to two weeks, and avoid strenuous activity for probably six weeks. Depending on your type of work, you can resume work after a few days or a long weekend. Wearing your surgical bra, and then transitioning to a strong underwire bra is a really important part of your recovery. These bras must be worn 24 hours a day for at least six weeks. This can seem like a real burden, but it’s worth it. These bras keep any stress from being placed upon your incisions. By being diligent with this your scars will stay as thin as is possible and will heal better. We usually need to move the nipples with this procedure, but we make every effort to maintain nipple sensation and breastfeeding function after breast reduction, but this is not always possible.

  What Is Surgical Body Contouring? After skin and tissues have been stretched out to accommodate a larger frame, they often lose their elasticity and are unable to “snap back” to where they were before the weight gain. Like a rubber band that has been stretched too far, the skin and tissue are unable to return to its former resistance and pliability. A body lift should be performed when the patient is as close to their desired goal weight as possible and has maintained a stable weight for at least 6 months. Since every patient has different needs and goals for their body type, a surgical plan would be mapped out during a free plastic surgery consultation. This plan may involve one stage of procedures, or various stages of procedures done months apart to allow adequate healing. What Are The Different Types Of Body Contouring Procedures? • Liposuction

  If you want to improve your shape by slimming areas of your body that exercise just can’t help, then liposuction might be your ideal procedure. Liposuction is a proven, effective solution for permanently removing stubborn areas of fat. The most common locations for this cosmetic body procedure include the neck (“double chin”), upper arms, abdomen, hips, and thighs. There are multiple ways to perform liposuction, but Watterson Plastic Surgery endorses and uses just a few: suction-assisted lipoplasty, ultrasound-assisted lipoplasty, and power-assisted liposuction. These different methods utilize the super-wet technique, which ensures fat is targeted more precisely, and post-operative bruising and swelling are minimized. The Liposuction Procedure here are several different techniques, cannulas and devices that can be used for liposuction. Some systems vibrate, others use water, still others use heat and of course many patients have heard of CoolSculpting which uses freezing to remove unwanted fat. The wonderful truth about liposuction is that you can “spot reduce” an area of fatty deposition. You can sculpt and contour which is almost impossible with exercise, diet or medications. To be able to “spot reduce” an area of unwanted fat is the incredible benefit of liposuction. The key consideration is the overlying skin and its elasticity. This is critical and makes for a wonderful result as compared to a mediocre or disappointing result. Liposuction is performed by first marking and identifying the area of concern. Then after adequate anesthesia, either local or general, the fat is removed via a cannula (tube) with suction. Myriad techniques abound, but I prefer using a technique labeled SAFE by a plastic surgeon in Louisiana. This means initial fat Separation and Fat Equalization. This particular technique uses a vibrating system to separate and then smooth the area after suctioning the fat away. The results are usually excellent and depend heavily on patient selection and site location. The key principle again is assessing the overlying skin and determining if the skin can tighten after removal of fat. Much like the skin tightens after pregnancy. Liposuction Treatment Options Indeed, liposuction is one of the most common surgical procedures performed in the United States, and it’s been that way for decades. As with any other surgical procedure, liposuction has advanced and evolved over the years. Today, there are a few cutting-edge practices that ensure optimal results while also emphasizing patient safety. Suction-Assisted Lipectomy (SAL) This more traditional form of lipectomy breaks up fat and uses a high-negative-pressure vacuum to remove the fat through a hollow instrument known as a cannula. The cannula is inserted through a small incision in the skin, often placed in inconspicuous areas to hide the visibly of any potential scars. Ultrasound-Assisted Lipoplasty (UAL) For fibrous body areas or larger amounts of fat, this technique involves inserting an ultrasound probe beneath the skin to “liquefy” the fat before it’s suctioned. This is particularly effective for the more stubborn fat deposits. Power-Assisted Liposuction (PAL) It’s also worth mentioning the emergence of PAL, or power-assisted liposuction. This is a procedure wherein the physician uses a vibratory device that is easy to control, thus allowing for a more precise and thorough fat removal process. PAL devices create quick, small vibrations that break up the fat into minuscule pieces, which are then easily suctioned away by the surgeon. While some trademarked techniques may be heavily promoted by others, they don’t always meet our standards at Watterson Plastic Surgery. Certainly, we understand the value of embracing the latest technology, but if a method doesn’t have proven success for our patients, then we won’t use it. That’s our promise to you. Will Liposuction Lead To Weight Loss? Many patients come to us asking if liposuction will help them lose weight. This is actually a very common misconception: liposuction is not about losing weight, and shouldn’t be considered as a weight loss solution. On the contrary, liposuction is all about body contouring. It’s all about redefining the shape of your body, not about your weight per se. Because of this, liposuction is generally recommended for those who live a healthy lifestyle. If you have a healthy diet and get regular exercise, yet still have problem areas that aren’t as contoured as you’d like, that might mean that you are a great candidate for liposuction. In fact, we generally feel that the best candidates for liposuction are those who are already fairly close to their ideal body weight. • Tummy Tuck

  What Is A Tummy Tuck? Clinically known as abdominoplasty, the goal of a tummy tuck is to tighten and change the contour of the abdomen area. The procedure removes excess fat and skin, and repositions weakened or separated muscles, resulting in a flatter, more contoured abdomen. Tummy tucks can have different lengths of incisions. Dr. Paul A. Watterson completely adapts the methods used for each of his tummy tuck procedures to the patient. What Are The Different Kinds Of Tummy Tucks? As mentioned above, Dr. Watterson uses different methods for his tummy tuck procedures based on the individual patient. Standard Abdominoplasty Also known as a full tummy tuck, Dr. Watterson makes two incisions. The first incision runs from hipbone to hipbone, usually running along the pubic hairline. The goal is to be able to cover the scar with a bathing suit. The second incision is made at the belly button. Once the skin is separated from the underlying muscle, the abdominal muscles are pulled together and stitched into place to make the abdomen firmer and to narrow the waist. This repairs stomach muscles that have actually become separated. Next, the skin is pulled downward, excess skin is trimmed away, and the navel reattached in a natural position. These full tummy tucks are usually the best option for women who have had children, as the localized weight gain stretches the stomach skin and separates the support muscles. Lipoabdominoplasty Or Saldanha Technique This is a newer technique which uses liposuction to thin the central abdominal tissues as well as tighten the skin of the abdomen. It was developed and promoted by Osvaldo Saldanha from Brazil. Dr. Watterson has used this method now for several years and the results are remarkable. Many patients want additional fat removed from the central abdomen as well as tightening loose skin and muscles. Umbilical (Belly Button) Float Or Transposition This technique is ideal for patients who still want to preserve a very natural belly button, and who have a small amount of loose skin or significant abdominal muscle diastasis or spread. In the right candidate, it is a wonderful procedure with very satisfying outcomes. Partial Abdominoplasty (Mini Tummy Tuck) Also known as a mini tummy tuck, in this procedure Dr. Watterson makes just a single incision along the pubic line. The length of this incision may vary depending on the degree of the patient’s sagging. Mini tummy tucks can be good for patients who don’t have a lot of excess sagging skin, but simply want to remove their stomach pooch. Circumferential Abdominoplasty (Extended Tummy Tuck) This method involves the same incisions as with a standard tummy tuck, but the bottom incision is extended. Where it usually stops at the hipbone in a standard procedure, here it extends around the flanks and onto the back. This allows Dr. Watterson to also address sagging, excess skin and fat on the outer thighs and buttocks. Fat and skin that are hanging over the belt line will be removed to create a slimmer, smoother contour. Dr. Watterson will tighten applicable muscle and will likely use some liposuction to remove unwanted pockets of fat. What’s The Difference Between A Tummy Tuck And Liposuction? The procedures are completely different in execution, and they really have different goals. Liposuction uses a small cannula with suction to remove stubborn pockets of fat that don’t respond to dietary changes or exercise. The cannulas are inserted through very small incisions and then break up and suction out the unwanted fat. The incisions made to gain access are tiny, usually needing only a bandage to close them. There is little recovery. But liposuction is only a good procedure for patients who still have good elasticity in their stomach skin. That’s because their skin needs to tighten down over their slimmer contour when the underlying fat is removed. If the skin is inelastic, the skin will be loose where the fat was removed. Tummy tucks are meant to remove excess sagging skin. They also seek to bring separated stomach muscles back together in the center of the stomach where they are anchored with sutures. Most tummy tucks also use at least a small amount of liposuction to remove small pockets of unwanted fat, but the main goal is the removal of sagging, loose skin. Tummy tucks require a fairly involved recovery due to the incision and the repositioning of muscles.

  What Is A Mommy Makeover? A mommy makeover is simply a term for a combination of surgical procedures that target the changes caused by pregnancy, delivery, and breastfeeding. At Watterson Plastic Surgery, our mommy makeovers involve a tummy tuck, breast lift with or without augmentation, and liposuction. These are combined into a single surgical session. Why Get A Mommy Makeover? While all women fully expect the life changes that come with having a child or children, few are ready for the changes that can occur to their figures. The localized weight gain and loss, both in the stomach and in the breasts, creates unique problems with stretched skin and sometimes separated muscles (in the abdomen). These are the procedures involved with our mommy makeovers: Breast lift (Mastopexy) – Because the breasts gain size and weight during pregnancy, the skin and support tissue can stretch, making the breasts flatten and sag. A breast lift removes excess skin and repositions the breasts back higher on the chest. Depending on the amount of sagging, the nipples may need to be relocated. Breast augmentation (Augmentation Mammoplasty) – Breastfeeding tends to decrease breast volume. Women who want to return volume, particularly to the upper part of the breasts, or who simply would like larger breasts, can opt to have breast implants inserted after the breast lift procedure. This is an easy time to add implants because they can be inserted through the incisions made for the breast lift. Liposuction – Stubborn pockets of fat can show up after pregnancy. They won’t usually respond to exercise, either. Liposuction removes these pockets on the inner and outer thighs, hips, and abdomen. Tummy tuck (Abdominoplasty) – As described above, the stretching of the stomach skin and muscles during pregnancy is extreme. A tummy tuck trims excess sagging skin and repositions separated muscles to give you back a flat stomach. Depending on the areas of sagging, a tummy tuck will involve one long incision from hipbone to hipbone positioned where it can be hidden beneath a bikini or underwear, and a second incision may be made around your navel. Candidates For Mommy Makeover Surgery First things first, a mommy makeover is not meant as a weight-loss procedure. Our Watterson Plastic Surgery patients need to be within 10 percent of their ideal body weight to qualify for these procedures. That is not an arbitrary goal — continued weight loss will create more sagging and loose skin, which is partially what these procedures are meant to address. Mommy Makeover FAQs Why Should I Combine All Of These Surgeries Into One Session? For anyone, the thought of combining three surgeries into one session seems overwhelming, and it is. But that’s also the beauty of a mommy makeover at Watterson Plastic Surgery. Yes, your recovery will be harder, because you’ll have three areas with incisions and tissue trauma. But you’ll only have one recovery, not the three you would have to deal with if you decided to have these procedures done separately down the road. My Post-Baby Body Is Bothering Me. When Can I Have A Mommy Makeover? It’s easy for a new mother to be frustrated with her new body, but a little patience is necessary. We advise our patients to have finished breastfeeding for at least three months before having these procedures. This allows the breasts to return to their normal, natural size. The other part of this timeframe is weight loss. You need to lose your baby weight to be ready for a tummy tuck, and this can take a few months.

The facelift is thought to be the original cosmetic surgery procedure, first performed in Berlin in 1901. Until around two decades ago, most of these procedures would be what is known today as a “traditional” facelift. But these methods often left the patient with a stretched appearance, as the skin was the focus of the procedure. Today, Dr. Watterson pays more attention to the underlying musculature and support tissues, lifting and bringing them back to their former higher position. This creates a more natural-looking rejuvenation. There are now a variety of different methods that Dr. Watterson uses for these procedures. He tailors every surgery to the individual situation of the patient. He may use a combination of methods. These are some of the common methods used: Traditional Facelift A full facelift seeks to provide the most rejuvenation across the mid-face down through the neck. A full facelift involves lifting and repositioning the deeper tissues in the face and removing certain attachments. Fat is sculpted (Dr. Watterson avoids removing too much fat to avoid creating a “hollow” look). Excess skin is trimmed and then re-draped. The incisions with a traditional lift start at the temples, run downward in front of the ear, wrap around the earlobe, and terminate in the hairline of the lower scalp. If the patient has particular issues in the neck area, a second incision is made under the chin to lift, tighten, and trim excess skin on the upper neck. Limited-Incision Facelift The goals of this facelift aren’t as widespread, instead providing limited rejuvenation around the eyes and mouth, including nasolabial folds and other deep creases. Shorter incisions are made at the temples, around the ears, and, if needed, in the lower eyelids and/or under the upper lip. Mid-Face Lift Volume loss in the cheeks is a common concern with many patients, and the mid-face lift targets those areas. Mid-face lifts can remove sagging in the cheeks, reduce puffiness under the eyes, and correct nasolabial folds. Incisions are small, usually in the lower eyelids or mouth. Neck Lift While not technically a “facelift,” neck lifts are often part of these procedures. After all, who wants a renewed face, only to have a sagging neck full of neck banding? The neck lift concentrates on jowls and fat under the chin. The incision is made around the earlobe and behind the ear to the lower scalp. An incision can also be made under the chin. Can I Combine A Facelift With Other Procedures From Dr. Watterson? Some patients are confused by the term “facelift.” They assume it reverses aging on the entire face, hence the name. However, a facelift actually only targets the mid-face down through the upper neck. Different muscles are involved in the forehead area and the lower neck. So, if you want more widespread rejuvenation, many of Dr. Watterson’s patients have a forehead lift and/or a neck lift done in coordination with their facelift.
